Student Withdraw During the School Year

If your student will be leaving Eagleview Middle School, we ask that you do the following at least one week before your student’s last day:

  1. The legal Guardian needs to contact the Registrar at 719-234-3436 to inform the Registrar of the student’s last day and provide the name and address of the school the student will be moving to.

  2. The legal Guardian needs to print the Notification of Withdraw form and complete/sign the front side and return it to the Counseling office.

  3. The counseling office will explain to the student how to complete the Check Out form (back side of Notification of Withdraw form). The student will then take the form to teachers and other staff for check-out grades, unpaid fees, borrowed books, etc.

  4. The student will bring the completed form (both sides) to the Counseling office.

Student Withdraw During the Summer

If your student will be leaving Eagleview Middle School for the upcoming school year we ask that you complete the Notification of Withdraw form (front side only).

You can return the completed/signed form by:

  • Dropping it off at front office of Eagleview

  • Emailing a scanned copy to the Registrar at deanna.traczek@asd20.org

  • Or mailing the form to:

Please make sure all school provided materials, such as library books and your student’s device (with case and charger) are returned to the school before your student’s last day. If these materials are not returned, you will be charged for the replacement of these items.
